Cholera Vaccines

CAUTION: Observe and exercise cautions for handling, preparing, administering, and disposing of the cholera vaccine. Inactivate any spilled vaccine and clean any non-disposable equipment used in the preparation of the cholera vaccine with 70% isopropyl alcohol or 10% bleach solution.

Oral Administration

Do NOT administer parenterally.
Do not mix with any other vaccines or solutions.
The patient may not have food or drink for 60 minutes before or after vaccine administration.
Prepare and administer the cholera vaccine in a health care setting equipped to dispose of medical waste.

Oral Liquid Formulations

If the packets are reconstituted in the improper order, the vaccine must be discarded.
Locate the 2 packets; the buffer component (Packet 1) and the active component (Packet 2).
Poor 100 mL of cold or room temperature (41 to 72 degrees F; 5 to 22 degrees C) bottled water (purified, spring, or sparkling [carbonated]) into a clean, disposable cup. Do NOT use tap/faucet water or any non-purified bottled water, other beverages, or other liquids.
Using scissors, cut off the top of the buffer component packet and empty contents into cup. Effervescence will occur.
Stir the buffer components with a disposable stirrer until completely dissolved.
Children 2 to 5 years: Discard half of the mixed buffer solution (total dose = 50 mL).
Using scissors, cut off the top of the active component packet and empty the active component packet contents (lyophilized V. cholera CVD 103-HgR) into the cup containing the buffer solution.
Stir for at least 30 seconds and until active component disperses to form a slightly cloudy suspension that may contain some white particulates. The active component may dissolve completely.
If desired, add no more than 4 grams (1 teaspoon) of sucrose (table sugar) or no more than 1 gram (1/4 teaspoon) of non-flavored stevia sweeteners. Once added, stir into the suspension. Do NOT add any other sweeteners or medicinal flavorings as this may reduce vaccine effectiveness.
Consume the cholera vaccine within 30 minutes, if sucrose or non-flavored stevia are added. Consume the cholera vaccine within 4 hours of reconstitution, if sucrose and non-flavored stevia have not been added, at room temperature around 70 degrees F (21 degrees C).
The recipient should drink the full contents of the cup at once. Some residue may remain in the cup and should be discarded with the cup.

For cholera prophylaxis against disease caused by Vibrio cholerae serogroup 01 in persons traveling to cholera-affected areas. Oral dosage Adults

100 mL PO as a single dose given a minimum of 10 days before potential exposure.

A duration of fewer than 14 days between stopping antibiotics and vaccination might also be acceptable in some clinical settings if travel cannot be avoided before 14 days have elapsed after stopping antibiotics.

The cholera vaccine contains live attenuated cholera bacteria that replicate in the gastrointestinal tract of the recipient. The exact immunologic mechanism by which the cholera vaccine protects against cholera gastroenteritis is unknown, however, rises in serum vibriocidal antibody 10 days after vaccination with the cholera vaccine were associated with protection in a human challenge study.

The cholera vaccine is administered orally. The vaccine is not absorbed systemically after oral administration.

Oral Route

After oral vaccine administration in adults 18 to 45 years, vaccine efficacy against the occurrence of moderate to severe diarrhea at 10 days post-vaccination was 90.3% (95% CI, 62.7%, 100%) and at 3 months post-vaccination was 79.5% (95% CI, 49.9%, 100%). After a dose of cholera vaccine, peak cholera shedding in stool occurred around day 7. In a study of 53 healthy adults, the percentage of recipients who shed vaccine on day 7 after the dose was 7.5% (95% CI, 2.1%, 18.2%) compared to 11.3% (95% CI, 4.3%, 23%) on any day through 7 days post-vaccination. The duration of shedding of the vaccine strain is unknown.

Pregnancy

The cholera vaccine is not absorbed systemically following oral administration and maternal use is not expected to result in fetal exposure to the drug. Maternal cholera disease is associated with adverse pregnancy outcomes including fetal death. The vaccine strain may be shed in the stool of the vaccinated mother for up to 7 days; transmission of the vaccine strain from mother to infant during vaginal delivery is possible. To monitor fetal outcomes of pregnant women exposed to the cholera vaccine, the manufacturer maintains a Vaxchora Pregnancy Registry. The cholera vaccine is not absorbed systemically by the mother following oral administration, and breast-feeding is not expected to result in exposure of the child to the vaccine strain.
